Copy the link and text in Practice Setting under Link to ACC form to appear in Cliniko communications, or just the link and use your own text, and paste into one or more of your appointment confirmations and/or one or more of one of your appointment reminders.
When a patient follows the link from the appointment confirmation/reminder, they will be taken to a blank form that they will have to fill in from scratch.
Patient not Found
If the patient is not found in SubmitKit or Cliniko this may because a change in name or different spelling you will need to find the patient that the form matches before importing the claim. Use the FInd Patient button on the Completed Forms page, that will show for not found patients.
